Dog and Puppy Application
The information requested in this form will assist us in matching you to the right dog for your family, needs and lifestyle. We
realize that some of the questions are intrusive, but we are sure you will understand that we are totally responsible for the puppies
we rescue as they have no one else to guarantee their lives and well-being. We take this responsibility seriously and will not
adopt out to any home when we cannot feel totally confident of the adopters willingness and ability (financial and personal) to
commit to this puppies future well-being and happiness.
An adoption fee is required at the time of the signing of the contract. This fee is non refundable for any reason. Throughout the
life of the puppy, if any reason should arise where you are unable to keep the dog, we require that you notify our rescue of the
availability of him/her and to return the adopted pet upon request. We strongly recommend that this puppy be micro chipped for
safety. Please realize that you are not at liberty to dispose of this puppy in any way, except to return her/him to us. The dog may
not be sold, given away, put in a shelter or euthanized without the knowledge and permission of rescue.
It is required that you take your new puppy to the vet within the 1st 48 hours of adoption.
Your new pet will come to you with all vetting documents: We still make no guarantee as to health and temperament.
Once the animal leaves our care it becomes your responsibility.
This Application is the first step in a process that we anticipate will culminate in the placement of a healthy and temperamentally
sound puppy into your home as a new family member.
Understand that the facility or property where adoptions are held is not responsible for anything that takes place or is adopted
from their property and should not be contacted in any case. The rescue holds the responsibility of what is adopted out.
Steps two and three respectively, will be a veterinary check (your veterinarian will be telephoned and asked about your previous
record of treatment and care of your pets) and a home check, where a member or representative of our rescue will visit your
home to ascertain the suitability of the living quarters for your puppy and to suggest any safety features that you may not be aware
of in providing a safe and secure home for your new puppy.
Adopt fees will be between $150 and 400 depending on vetting done. Adoption fee will be collected at the time of delivery of your
dog. Cash, visa and master card are the only form of payments accepted. No checks due to the amount of bad ones that have
been received. There is a convenience charge of $8.98 to use visa and master card.
No information offered on this form by you will be given to third parties for any purpose.
We reserve the right to refuse adoption to anyone, for any reason, at any time, in accordance with our experience and sense of
responsibility for the puppies in our care.

Any dog we pull is taken from the shelter to the vet for all their shots, heart worm test (if age appropriate), treated for fleas, fecal tested, de wormed, and
spay/neuter
Here's how our fees are broken down
$15 Pull Fee per Dog (depends on shelter)
$35 Health Certificate (Legal requirement for inter-state travel, require a complete exam and state documentation. This is passed on to the adopter, and we keep
it for our records)
$30-$50 Shots
$35 Microchip
$18 Fecal Testing
$15-$30 De wormer (we use the BEST de wormer's (panacur, strongid) and pro actively prevent giardia)
$35 Heart worm Testing
$90-$150 Spay/Neuter
$15-$30 External Parasite Control (Comfortis)
$10 each tablet of Heartworm preventative (Iverhart Max)
$200-$800 to treat Heart worm if the dog is positive, sometimes more
Our adopt fee for a fully vetted dog / puppy is $270-$400
Please note that many dogs/puppies have worms and are de wormed once or twice prior to transport. You will need to follow up with another de worming. Our
suggestion is to bring your dog to the vet within 48 hours of receiving for an exam. Also, the above list is our standard, but some dogs may not receive all
these thing or may receive more. It depends on the shelter.
Adopting is a huge responsibility and it needs to be for the life of the animal. Saving Fur Kids only will adopt to forever homes and it is mandatory that if a dog
needs to be returned it comes back to us. We do not tolerate any of our dogs going into a shelter. We do not rescue them from high kill shelters for them to end
up anywhere except loving, forever homes. We take this oath seriously and expect all our adopters to do the same. Adopting should be a lifelong commitment and
an exciting experience!
